Move, Add and Changes (MAC) Forms
Please find below a range of 2degrees Managed Services MAC forms, used for requesting changes to your existing services as required.
All the .pdf forms are editable and when completed should be sent to the email address listed in the Contact Details.
The following applies to Simple MAC requests:
- The MAC request can be carried out remotely by a Managed Services engineer and does not require attendance at a customer's site.
- A Simple MAC is a request for changing the characteristics within an existing service or services for a customer.
- A Simple MAC request can be applied up to 5 changes of the same type within the existing service and the effort to complete the request is no more than 4 hours.
- The Simple MAC request does not require a planned outage event or notification.
The following applies to Complex MAC requests:
- A complex MAC is a request for changing the characteristics of an existing service or services that has an effort of work grater than 4 hours.
- A complex MAC can be for multiple changes on multiple devices or services.
- A quote is required to scope the effort required to complete the work and this will be provided within 2 business days of receiving the request from the customer.
- An approved Purchase Order is required from the customer before the work can commence on a complex MAC.
The following are exceptions to the MAC process:
- Any request that requires a product change, price or contract change to the monthly billing or a solution design change, these types of requests are outside of the scope of the MAC process.
- These types of requests should be directed to your Account Manager or SDM.
- Any urgent complex MACs will be reviewed by the Managed Services team and handled on a case-by-case basis.
